To get a PhilHealth ID, first get your PhilHealth Identification Number (PIN) by registering with the PMRF and a valid ID or PSA birth certificate: through your employer, online on the PhilHealth Member Portal (PIN emailed in 3 to 5 working days), or at a PhilHealth office. PhilHealth then issues your Member Data Record (MDR) and PhilHealth ID card. There's also an optional PVC Health Insurance Card for ₱90, which PhilHealth says is a valid government-issued ID.

PhilHealth ID vs PIN vs MDR

What it isHow you get it
PIN (PhilHealth Identification Number)Your permanent member number. You use it for every PhilHealth transaction, for lifeWhen you register (online, at an office, or through your employer)
Member Data Record (MDR)A printout of your membership record, including your declared dependentsIssued after registration; you can view or print it in the Member Portal
PhilHealth ID cardA card showing your name and PINIssued with your MDR after registration (employees get it through their employer)
PVC Health Insurance Card (HIC)An optional plastic cardVoluntary, ₱90, per PhilHealth Advisory 2016-0012

The PIN is what matters. Employers, hospitals and PhilHealth all work with your PIN. The card just shows it.

How to get a PhilHealth ID

Step 1: Get your PIN (if you don't have one)

  • Employees: fill out the PhilHealth Member Registration Form (PMRF) and give it to HR. Your employer gives you your MDR and PhilHealth ID card.
  • Everyone else: register online on PhilHealth's Online NHIP Registration (upload your PMRF and a birth certificate or valid ID; your PIN is emailed in 3 to 5 working days), or walk in at a Local Health Insurance Office (LHIO) or PhilHealth Express.

Full steps and requirements: PhilHealth registration.

Step 2: Get your ID card

PhilHealth's walk-in steps end with "await Member Data Record (MDR) and PhilHealth ID card". If you registered online and only have your PIN, go to an LHIO or PhilHealth Express (office directory) with your PIN and a valid ID and ask for your ID card and MDR. Bring a filled-out PMRF too, since it's the form PhilHealth uses for registration and updates.

Requirements

  • PhilHealth Member Registration Form (PMRF) (download), two copies if you register in person
  • A valid ID or PSA birth certificate (PSA e-Certificates are accepted under PhilHealth Advisory 2026-0051)
  • Your PIN, if you're already registered
  • For the PVC card: the ₱90 fee

PhilHealth's registration pages don't list a fee for registering or for the regular ID card. The ₱90 applies to the optional PVC card.

Is the PhilHealth ID a valid ID?

The PVC card, yes. PhilHealth Advisory 2016-0012 says the PVC-type Health Insurance Card, issued "on a voluntary basis at a cost of Php 90.00", is a "duly recognized valid government-issued identification card" that should be honored wherever identity has to be verified (under BSP Memorandum M-2012-021). It can also be shown at accredited hospitals instead of your MDR, together with Claim Form 1.

In practice, offices decide which IDs they accept, and many treat the PhilHealth ID as a secondary ID. The DFA, for example, does not accept it for passports (see passport requirements). Check our valid IDs list before you go.

Find your PIN (your PhilHealth number)

If you're already registered but lost your number:

  • Check your Member Data Record (MDR) or an old PhilHealth ID card.
  • Employees: ask HR. Your employer reports your PIN in its remittance reports (the ER2), so payroll has it.
  • Visit any LHIO or PhilHealth Express with a valid ID, or call the PhilHealth Corporate Action Center at (02) 8662-2588.

Don't register again to get a new number. Your PIN is permanent, and PhilHealth's online system allows only one application per person.

How to get or print your MDR

Your Member Data Record (MDR) is your PhilHealth membership record, including your declared dependents. PhilHealth lists these ways to get it:

  • Online: log in to the PhilHealth Member Portal, where you can view or print your MDR.
  • Employees: your employer gives you your MDR and PhilHealth ID card after you register through HR.
  • Walk-in: registrations and updates at a PhilHealth office end with a printout of your MDR. Bring a valid ID.

Check that your dependents are on it. PhilHealth says your dependents should be listed on your MDR for hassle-free benefits, including registering them for PhilHealth YAKAP.

Using your PhilHealth ID

  • Give your PIN to every new employer, so your contributions go to the right record.
  • In the PhilHealth Member Portal you can view your records and contributions, pay premiums online, view or print your MDR, and choose your YAKAP clinic.
  • At the hospital, PhilHealth benefits are claimed with your PIN and Claim Form 1. Self-paying members may be asked for proof of payment.
  • Keep your premiums paid. See PhilHealth contribution 2026 and the contribution calculator.

Update the details on your ID

Changed your name or civil status, or found a wrong birth date? Fill out the PMRF, tick "Updating/Amendment", attach supporting documents (for example a PSA marriage certificate), and submit it at a PhilHealth office. You'll get an updated MDR. See updating your records.

Beware of fixers. Register only through the PhilHealth Member Portal, PhilHealth offices or your employer. Never pay someone online to "get your PhilHealth ID" for you.

Frequently asked questions

How do I get a PhilHealth ID?

Register first to get your PhilHealth Identification Number (PIN): through your employer's HR, online on the PhilHealth Member Portal, or at a PhilHealth office, with the PMRF and a valid ID or PSA birth certificate. PhilHealth then issues your Member Data Record and PhilHealth ID card.

What are the requirements for a PhilHealth ID?

A filled-out PhilHealth Member Registration Form (PMRF) and a valid ID or PSA birth certificate. If you're already registered, bring your PIN and a valid ID. The optional PVC card costs ₱90.

How much is the PhilHealth ID?

PhilHealth's registration pages don't list a fee for registering or for the regular ID. The optional PVC Health Insurance Card costs ₱90, per PhilHealth Advisory 2016-0012.

Is the PhilHealth ID a valid ID?

PhilHealth says its PVC Health Insurance Card is a valid government-issued ID (Advisory 2016-0012). Each office still decides what it accepts; many treat it as a secondary ID, and the DFA doesn't accept it for passports.

Can I get a PhilHealth ID online?

You can get your PIN online through the Member Portal (emailed in 3 to 5 working days). For the physical ID card, go to a PhilHealth office, unless you're employed, in which case your employer gives it to you.

Does the PhilHealth ID expire?

Your PIN is permanent and never changes. PhilHealth's pages don't give an expiry date for the ID card.

How do I find my PhilHealth number?

Check your Member Data Record or old ID, ask your employer's HR (they report your PIN to PhilHealth), or visit a PhilHealth office with a valid ID. You can also call (02) 8662-2588. Don't register again: your PIN is permanent.

I lost my PhilHealth ID. What should I do?

Your number stays the same, so the card is just a copy of it. Visit an LHIO or PhilHealth Express with a valid ID to ask for a replacement and a copy of your MDR. PhilHealth doesn't publish a separate online procedure for lost IDs.
