Which way to register
| You are… | Where to register | What you get |
|---|---|---|
| Newly hired employee (private or government, kasambahay, family driver) with no PIN yet | Fill out the PMRF and give it to your employer's HR | Your employer gives you your Member Data Record (MDR) and PhilHealth ID card |
| Employee who already has a PIN | Just tell your employer your PIN (they report it in their ER2) | – |
| Self-earning, freelancer, professional, informal worker | Online on the Member Portal, or walk in at an LHIO or PhilHealth Express | PIN, MDR and PhilHealth ID card |
| Land-based OFW, Filipino living abroad, dual citizen | Online, or at an LHIO or PhilHealth Express | PIN, MDR and PhilHealth ID card |
| Student, unemployed or first-time job seeker | Online, or at an LHIO or PhilHealth Express | Your PIN, ready for your first employer |
Already registered? Don't register again. Your PIN is "unique and permanent", says the PMRF, and you use the same number for life, even when you change jobs or go abroad. See how to find your PhilHealth number.
Requirements
- PhilHealth Member Registration Form (PMRF), fully filled out (download from PhilHealth). Walk-in registrants fill out two copies.
- Proof of identity: a valid ID or your PSA birth certificate. PhilHealth's online registration asks for "birth certificate, valid, acceptable, or duly issued ID".
- For self-earning members, kasambahay and family drivers: your actual monthly income on the PMRF. Self-earning members and professionals also submit proof of income.
- To declare dependents: documents that prove the relationship, such as PSA birth or marriage certificates.
PSA e-Certificates are accepted. PhilHealth Advisory 2026-0051 (23 September 2026) says PSA e-Certificates (birth, marriage, death, CENOMAR) must be accepted for registration, updating and claims, as a clear printout at the office or a clear digital copy uploaded to the Member Portal. See how to get a PSA birth certificate.
PMRF 2026: the PhilHealth Member Registration Form
The PMRF is the one form behind every PhilHealth registration and record update. When we checked PhilHealth's Downloads page on 26 October 2026, the form posted there was still the PMRF "UHC v.1 January 2020": there is no separate 2026 version, so that's the one to use.
| Form | Who uses it | Official file |
|---|---|---|
| PMRF (PhilHealth Member Registration Form) | Filipino members registering for a PIN or updating their records | pmrf_012020.pdf |
| PMRF-FN (for Foreign Nationals) | Foreign nationals registering with PhilHealth | PMRF_ForeignNatl.pdf |
How you use the PMRF depends on how you register:
- Online: you fill out the PMRF on the Member Portal and upload it with your birth certificate or valid ID (steps below).
- Walk-in: print and fill out two copies and bring them to a PhilHealth office.
- Newly hired employees: give the filled-out PMRF to your employer's HR.
- Updating records: use the same PMRF and tick "Updating/Amendment" (see updating your records).
On the form you also pick your member type. Direct contributors include employees, kasambahay, self-earning members, migrant workers and lifetime members; indirect contributors include senior citizens and sponsored members. Tips for filling it out are in common PMRF mistakes.
How to register online (Member Portal)
Open PhilHealth's online registration
at memberinquiry.philhealth.gov.ph ("PhilHealth Online NHIP Registration – Individual").
Enter a valid email address
It can be used only once, and you can submit only one application.
Fill out the PMRF online
with your name exactly as it appears on your birth certificate.
Upload your PMRF and supporting document
, such as your birth certificate or a valid ID. PhilHealth says applications without complete documents "will be disregarded".
Wait for the email
Complete applications are processed in 3 to 5 working days, and your PIN is emailed to you.
After you have your PIN, you can get your PhilHealth ID card and Member Data Record (MDR). See the PhilHealth ID guide.
How to register in person
PhilHealth's steps for the informal economy:
- Go to any Local Health Insurance Office (LHIO) or PhilHealth Express outlet (office directory).
- Fill out two copies of the PMRF.
- Submit the PMRF with your ID or birth certificate.
- Get your Member Data Record (MDR) and PhilHealth ID card.
- Pay your premium using your PhilHealth number (self-paying members). See PhilHealth contribution 2026.
Filling out the PMRF: common mistakes
From the instructions on the back of the PMRF (UHC v.1, January 2020):
- Write in CAPITAL LETTERS. Put "N/A" in anything that doesn't apply.
- Use your name exactly as on your birth certificate, and your mother's full maiden name as it appears there.
- Tick the right purpose: "Registration" for a new PIN, "Updating/Amendment" to change existing records.
- Choose your member type (employed private, employed government, kasambahay, self-earning, migrant worker, and so on).
- Self-earning members, kasambahay and family drivers write their actual monthly income. Other direct contributors (except employees, sea-based OFWs and lifetime members) also write their profession and proof of income.
- Mobile number is required. Email is required for OFWs.
- PhilSys ID number and TIN are optional.
Declaring dependents
Dependents are covered without extra premiums, but they must be listed on your MDR. PhilHealth's qualified dependents are:
- your legitimate spouse who isn't a member;
- children below 21, unmarried and unemployed (legitimate, legitimated, acknowledged, illegitimate, adopted or stepchildren), and children 21 or older with a disability that makes them totally dependent on you;
- foster children under RA 10165;
- parents aged 60 or above who aren't members themselves, and parents with a permanent disability that makes them totally dependent on you.
Dependents with a disability are registered as principal members under RA 11228 (mandatory PhilHealth coverage of persons with disability).
Updating your records
To change your name, civil status, address, contact details or dependents, or to correct your birth date or sex:
- Download the PMRF and tick "For Updating" (Updating/Amendment).
- Fill out only what's changing, and attach supporting documents (for example a PSA marriage certificate for a change of civil status).
- Submit it at the nearest PhilHealth office.
- Wait for the printout of your updated MDR.
Changing jobs, going abroad or retiring
From PhilHealth's "continuing your coverage" page:
- New employer in the Philippines: give them your existing PIN.
- Leaving as a land-based OFW: use your existing PIN when you register and pay as a migrant worker.
- Retirement age, at least 120 monthly premiums paid and no longer working: register as a Lifetime Member (age 60 and above; 56 for uniformed personnel).

Frequently asked questions
What are the requirements for PhilHealth registration?
A filled-out PhilHealth Member Registration Form (PMRF) plus a valid ID or your PSA birth certificate. Self-earning members also declare their monthly income and submit proof of income. Walk-in registrants fill out two copies of the PMRF.
Can I register with PhilHealth online?
Yes. Use PhilHealth's online NHIP registration on the Member Portal (memberinquiry.philhealth.gov.ph). Upload your PMRF and a birth certificate or valid ID. Complete applications are processed in 3 to 5 working days and your PIN is emailed to you.
How long does PhilHealth registration take?
Online registration takes 3 to 5 working days for complete applications, per PhilHealth. Employees get their MDR and ID through their employer.
Can I register with PhilHealth even if I'm unemployed or a student?
Yes. Register online or at a PhilHealth office as an individual so you already have a PIN when you get your first job. Your employer will then use that same PIN.
I'm newly hired. Do I register myself?
If you don't have a PIN yet, fill out the PMRF and give it to your HR department. If you already have a PIN, just give your employer the number.
Can I have two PhilHealth numbers?
No. Your PIN is unique and permanent, and the online system allows only one application per person. If you already registered, find your existing PIN instead of registering again.
Are PSA e-Certificates accepted for PhilHealth registration?
Yes. PhilHealth Advisory 2026-0051 directs all PhilHealth offices to accept PSA e-Certificates for registration, updating and claims, as a clear printout or a clear upload on the Member Portal.
Is there a new PMRF for 2026?
No. When we checked on 26 October 2026, PhilHealth's Downloads page still posted the PMRF marked "UHC v.1 January 2020" (pmrf_012020.pdf). Foreign nationals use a separate PMRF-FN. Download them only from philhealth.gov.ph.
Where can I download the PMRF?
From PhilHealth's own Downloads page (philhealth.gov.ph/downloads), under Membership: the PMRF for Filipino members and the PMRF-FN for foreign nationals. You can also fill out the PMRF online when you register on the PhilHealth Member Portal.
Can first-time job seekers and students register with PhilHealth online?
Yes. Students, unemployed people and first-time job seekers can register online on the Member Portal or at a PhilHealth office. You get your PIN, which your first employer then uses.
How do I add my spouse or child as a dependent?
Fill out the PMRF, tick "For Updating", list the dependent, and attach proof of the relationship (for example a PSA marriage or birth certificate). Submit it at a PhilHealth office and get your updated Member Data Record.
