PSA Online Appointment: How to Book a Slot at a PSA Outlet

PSA outlets don't take walk-ins. To get a birth, marriage or death certificate or a CENOMAR over the counter, you need an appointment first. It's free and takes a few minutes. Here's how, step by step, plus where the outlets are.

Book on the PSA's Civil Registration Service (CRS) Appointment System (crs-appointment.psahelpline.ph): choose the purpose and the PSA outlet, enter the name, email and mobile number of the person who will go, verify the one-time PIN sent by email, pick the document, then the date and time. The appointment slip is emailed to you. Booking is free and the slip is non-transferable. Arrive 30 minutes early with the slip, a valid ID and any authorization letter. Fees are paid at the outlet: ₱155 per birth, marriage or death certificate, ₱210 per CENOMAR.

If you only need a copy of your own certificate and can't get a slot, ordering online is often faster.

Booking is free. Don't pay for a slot. Search results for "PSA online appointment" are full of Facebook and Instagram pages selling "appointment assistance" for ₱50 or more. You don't need them, and the PSA says to transact only with authorized PSA personnel. Book it yourself with the steps below.

How to book a PSA online appointment

From the PSA's CRS Appointment System:

  1. Open the CRS Appointment System

    and read the steps, the privacy notice and the important reminders.

  2. Select the purpose of the appointment

    If it's for a court decree or legal instrument (for example, the first copy of an annotated certificate), you must book at East Avenue, Quezon City.

  3. Select the PSA outlet

    and read its reminders.

  4. Enter the appointee's details:

    the complete name, active email and mobile number of the person who will go to the outlet in person. Use the name exactly as it appears on that person's ID.

  5. Verify the one-time PIN

    sent to the email address.

  6. Select the documents

    If you're requesting for someone else, choose your relationship and enter the owner's details. A representative can request for only 2 people.

  7. Pick the date and time

    Greyed-out slots are full.

  8. Review and confirm

    The appointment slip is emailed to you. Print it or keep it on your phone.

  9. Go to the outlet a little early

    with the completed application form, valid IDs and any authorization letter, SPA or affidavit.

No slots? New slots open over time, so check again, especially in the morning. Or order online instead.

Missed it or made a mistake? PSAHelpline says confirmed appointments can't be rescheduled and a missed slot means no entry, so book a new appointment with the correct details.

What to bring on the day

  • Your appointment slip, printed or on your phone. You show it to the guard or information marshal.
  • A valid ID whose name matches the booking, for example a PhilID or ePhilID, passport, driver's license, UMID, PRC ID, postal ID, voter's ID, senior citizen or PWD ID, or an NBI or police clearance. See the full list of valid IDs.
  • An authorization letter or SPA and the owner's ID, if you're requesting a certificate that isn't yours and you're not the owner's parent, spouse, direct descendant or guardian.
  • Cash for the fees: ₱155 per birth, marriage or death certificate (copy issuance, authentication or annotation) and ₱210 per CENOMAR.
  • Your own pen, to fill in the application form.

  4. Claim the certificate at the releasing area on the release date and time, and check it before you leave.

First copies of certificates with court decrees or legal instruments are processed only at the PSA East Avenue outlet in Quezon City, so book that outlet.
